Guide to Toothache That Worsens at Night: What Pain That Wakes You Means and How to Cope Overnight
If a toothache gets worse at night and wakes you or keeps you from sleeping, first suspect irreversible pulpitis, in which the tooth nerve has inflammation that is hard to reverse. Even if a painkiller settles it for a while, the cause remains, and root canal treatment or extraction may be needed. If the pain repeats at the same time of day, neurovascular toothache that comes with migraine is also considered.
Key facts

What causes are suspected when a toothache worsens at night?
The KDCA National Health Information Portal lists, as features of irreversible pulpitis, pain that continues even after the stimulus stops and spontaneous pain that hurts even at rest. This kind of pain can appear at night too, waking you from sleep or disturbing it.
The pain of irreversible pulpitis can spread to the ear or cheek but does not cross the midline of the face. Patients often cannot point exactly to the causative tooth, so it is not strange to be unsure which tooth, upper or lower, it is.
Neurovascular toothache that accompanies migraine can have the feature of repeating at the same time of day on a daily, weekly or monthly cycle. If it starts at a similar hour each night and a headache comes with it, mention this possibility at the visit too.
What symptoms should be checked along with night toothache?
Checking the following helps narrow down the cause at the next day's visit. It is also a good method to jot down a short note with the time when symptoms occur.
If the face or gum swells and a fever develops or swallowing is hard, it may not be a simple night toothache but a sign that infection is spreading, so do not wait until morning.
- Whether the pain lingers long after eating something cold or hot
- Whether it hurts more when chewing or biting the teeth together
- Whether the gum is swollen or pus is visible
- Whether a headache, stuffy nose or chest tightness are also present
- Whether it begins at a similar time every day
What can I do at home when it hurts at night?
Acetaminophen is a fever reducer and painkiller whose MFDS-approved indications include toothache. Adults can take 0.5-1.0 g per dose, 3-4 times a day, up to 4 g a day, and first check whether other cold medicines or painkillers contain the same ingredient.
Do not chew on the painful side, and before sleeping avoid hard food or very cold or hot food. If food is stuck, brush gently to remove it, but do not scrub the painful area hard.
Even if a painkiller settles the pain and you fall asleep, book a dental visit for the next day, because a painkiller only relieves symptoms and does not treat the cause of inflammation.
When taking medicine at night, noting the time and amount helps you avoid exceeding the daily total if it hurts again before dawn. Children's syrup is set to be given at intervals of 4-6 hours and not more than 5 times a day, so be careful not to give it many times overnight.
How do I find care at night or on holidays?
Hospitals, clinics and pharmacies open late at night can be searched by location and department on the Emergency Medical Portal e-gen website or app, and you can also phone 129 or 120 to ask. This is the method the Ministry of Health and Welfare announces each holiday.
Even for a place that is open, hours can change, so confirm by phone before setting out. If symptoms are mild, the government recommends first using an open neighborhood hospital or clinic rather than the emergency room.
However, if the face and neck are swollen and breathing or swallowing is hard, or there is chest pain with the toothache, do not wait; call 119 or go to the emergency room.
If the pain is gone by morning, can I put off treatment?
It is better not to put it off. The KDCA explains that when nerve necrosis progresses in irreversible pulpitis, the pain sometimes lessens on its own within days. The pain going away does not mean it has healed.
If nerve infection is left alone, bacteria cause pain again, and inflammation can spread through the root tip to the surrounding bone and progress to conditions such as osteomyelitis. If it hurt enough to wake you even once at night, get care even if the pain is gone.
If root canal treatment is started because of night toothache, you will usually have several more visits at intervals of 3-7 days. If night pain becomes severe again during treatment, or the temporary filling comes out, do not wait for the next appointment; contact the dental clinic treating you.
Common myths and facts
Myth: If it does not hurt in the morning, it healed on its own.
When nerve necrosis progresses, pain can lessen, but without treatment inflammation can spread to the root tip and bone.
Myth: A toothache that hurts every night is always caused by a cavity.
Toothache that repeats at the same time of day may be neurovascular toothache accompanying migraine, so the cause is looked at along with a dental examination.
Frequently asked questions
My toothache seems worse when I lie down.
The typical cause of toothache at night is irreversible pulpitis with spontaneous pain. The relationship between lying down and pain differs from person to person, but if it is bad enough to wake you every night, nerve inflammation may have progressed, so get dental care soon.
If it still hurts after taking a painkiller at night, can I take more?
The MFDS advises not to increase the dose on your own because it is not working. Keep to the set dose, and if it is still hard to bear, find an open medical institution or consult a doctor or pharmacist.
Does holding ice in the mouth at night help?
The response to cold stimulation depends on the cause and stage of the toothache, so it is hard to say whether ice will help. The KDCA advises not chewing hard things like ice for cracked tooth care, so do not bite ice. Ask the dentist about compress methods when there is swelling.
If I go to the emergency room for night toothache, can I get dental treatment?
The emergency room centers on controlling pain and infection, and treatment of the cause, such as root canal treatment, is usually continued at a dental clinic. If there is no swelling, fever or difficulty swallowing, it is usual to use the dental clinic the next day.
My child cries at night saying a tooth hurts.
Children can also feel pain at night if decay is deep. Children's acetaminophen is set within a range of 80 mg to 640 mg per dose by age and weight, so check the product label, and see a dentist when day breaks.
